MRI : Synovial thickening and increased enhancement of the atlantoaxial joint, anterior atlantodental interval widening. Bony erosion of the odontoid process posterior cortex B. 나중에 찍은 plain film: anterior atlantodental interval widening(3.4 mm, 정상은 3 mm 미만), left wrist bone erosion, joint space narrowing Differential Diagnosis: infection, trauma Diagnosis: rheumatoid arthritis involving atlantoaxial joint Discussion: atlantoaxial joint는 synovial joint로, rheumatoid arthritis 환자에서 비교적 일찍 이환된다. 즉 dens와 C1, transverse ligament 사이의 synovium에 inflammation이 생겨서 pannus가 형성되고 이로 인하여 transverse ligament가 손상되어 instability가 발생한다. Subluxation은 70% 이상의 rheumatoid arthritis 환자에서 일어나나, dislocation은 약 25%에서 발생한다. Pannus와 erosion에 따른 bone destruction의 결과로 spinal cord compression이 생길 수 있으며 때로 odontoid process 전체가 erosion 되어 소실되기도 한다. References: |
