Discussion

Findings:

On plain radiography,
irregular endplate changes with sclerosis involving T8, T12 bodies are shown.
On CT, prominent sclerosis involving left clavicle is noted. T-L spine MR shows
osteitis in the multiple TL spine. Findings of PVP are shown in the L3 and L4.

Diagnosis:
SAPHO

Discussion:

•       A variety of subacute and chronic
osteomyelitis of unknown cause that occurs in childhood and that frequently
reveals multiple and symmetric alterations.

•       The usual age of onset of the disease
is 5 to 10 years, although infants and adults may be affected.

•       Synovitis, acne, pustulosis, hyperostosis,
osteitis --> osteolysis
with bone sclerosis (dominant radiographic finding)

•       Cartilaginous joints involvement such as discovertebral
junction & symphysis pubis

•       Synovial joints such as sacroiliac
joint

- often
unilateral

•       Bone lesions may occur in the absence
of any skin lesions.
