Aneurysmal bone cyst

Discussion

Findings:

A: Humerus latateral view show well defined exponsile osteolytic lesion without sclerotic margin in proximal shaft of the right humerus.

B: Axial MR image show well defimed slight expansile low signal intensed lesion on T1WI (B1) fluid-fluid leveled high and low signal intensed lesion on T2WI (B2) and marginal enhancement on T1- enhanced image (B3).

C: Sagittal MR image show lobular margined low signal rimed fluid-fluid leveled high and low signal intensed lesion and around bone marrow edema on T2WI (C1) and marginal enhancement and around bone marrow enhancement on T1-fat sat enhancement image (C2).

Diagnosis: Aneurysmal bone cyst

Discussion:

Expansile, thin-walled, blood filled cyst로 20세까지 80%가 생김. 70%가 원발성이며                  chondroblastoma, chondromyxoid fibroma, osteoblastoma, giant cell tumor, fibrous dysplasia, osteoblastoma,  chondrosarcoma의 속발성으로 올 수 있다. Long tubular bone에서 70%가 발생하며 주로 metaphysis에 발생하며, 일반방사선 사진에서는 geographic, eccentric, expansile radiolucency 을 보이며 MRI상 T1WI에서 low signal intensity, T2WI에서 high signal intensity를 보인다. Fluid-fluid level 이 보이면 진단에 도움이 될 수 있다.
