Diagnosis:

tethered cord syndrome



Discussion




Findings: Sagittal T1W and T2W scans show a low-lying tether
cord with fibrolipoma inserting into a terminal lipoma. Lipoma is contiguous
with subcutaneous fat through dorsal dysraphism.



 



 



Differential Diagnosis: no



 



Diagnosis:
low conus (tethered conus, tight filum
terminale, cord traction) syndrome



Discussion:



 Genetal features

Conus ends
below L2 inferior endplate; tethered by thickened filum with/without
fibrolipoma, terminal lipoma



-      
Location;
T-L junction à sacrum



-      
Size; filum
thickened (>2mm at L5-S1, axial MRI)



-      
Scoliosis
(20%)에서 동반



Clinical
issues



-      
Lower back
pain and leg pain; worst in morning, exacerbated by exertion



-      
Urinary
bladder dysfuntion
