Diagnosis:

Os acromiale with RCT

Discussion

Findings:
Shoulder axillar view; Unfused ossification center of distal end of acromion
MRI ; 1. Os acromiale on T1-weighted axial scan and T2-weighted image sagittal scan
2. Rotator cuff tear and retraction with fluid collection in subacromial-subdeltoid bursa on T2-weighted coronal and sagittal scan

Discussion:
A persistent ossification center at the free end of the acromion occurs in as many as 15% of persons. Pain and higher prevalence of the shoulder impingement syndrome and tears of the rotator cuff have been associated with an os acromiale. Its appearance simulates that of a fracture of the acromion
